Vaccination Information

Here you will find information related to the vaccination requirements and process for all incoming students.

Chatham requires that all incoming students provide their vaccination records before the first day of classes. Failure to provide your records will prevent you from enrolling for subsequent semesters.

Students whose programs are completely virtual are not required to upload immunization documentation.

Required Vaccinations

For students matriculating after January 1, 2024 the following vaccinations are required:

  • 2 doses MMR vaccine or positive titers for Measles, Mumps, and Rubella 
  • 2-3 doses Hepatitis B vaccine (depending on the type of vaccine) or a positive Hepatitis B surface antibody blood-test 
  • 1 dose Meningitis A vaccine (1 dose is required AFTER age 16) 
  • 2 doses Varicella vaccine or positive titer 
  • Tuberculosis screening questionnaire (Follow-up testing required if screening is flagged) 
  • Some graduate health science programs may require additional vaccinations for clinical setting requirements. Check with your program.  

Other vaccines, including Tdap (Tetanus, Diphtheria, and Pertussis) vaccine, Hepatitis A vaccine, HPV vaccine, and yearly flu and Covid-19 vaccinations are highly recommended but not required. Submitting Vaccination Records

Graduate students in the College of Health Sciences will submit their vaccination records through the Exxat One System directly with their graduate program to satisfy Chatham’s vaccination and clinical site vaccination requirements.* 

All other undergraduate and graduate students (living on campus or off) will submit their vaccination records through the Med+Proctor System. Med+Proctor is an online immunization verification tool used to track students' vaccinations and compliance. Med+Proctor provides immunization certificates to download and give to your PCP to fill out. It is not required to submit immunizations on the certificate, as the certificate is there only if you'd like to use it. Waivers are also available for download underneath the documents section. 

*College of Health Science graduate students planning to live in on-campus housing will need to submit vaccination records through both Exxat and Med+Proctor.

Med + Proctor Vaccine Upload Instructions

  1. Visit secure.medproctor.com and click New Users-Register 
  2. Register with your Chatham email address. 
  3. When approaching the documents section, if asked to pay for a Pro Account, click No Thank You. Med+Proctor is a free service, and the Pro account is not needed at any point during this process. 
  4. Chatham requires students to upload the immunization records for all required vaccines.
